29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:      On or about May 4, 2015, employees were exposed to fall hazards of approximately 13 feet in height to the paved ground below from climbing the scaffold end frames and cross braces to access the scaffold while performing bricklaying work on a commercial building under construction.

29 CFR 1926.451(h)(1): In addition to not wearing hardhats each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, or guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects:    On or about May 4, 2015, employees were exposed to a struck by hazards from falling masonry tools and rocks while performing brickwork without hard hats from a scaffold that did not have toe boards installed.
