29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items (a) through (d) of this section: On or about April 29, 2015, a lockout procedure which clearly defined the specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy did not include instructions regarding isolating the small turning auger of Mini Prep II where exposure to sharp blades occurred through the open port hole.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: On or about April 29, 2015, an employee was exposed to the turning sharp blades of the small auger in Mini-Prep 2 department at the open port hole of the condiment loader when he was not trained as an authorized lockout employee to deenergize and lockout the auger before cleaning away meat and condiment refuse.
