29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports:  On or about May 4, 2015, employees were exposed to fall hazards of approximately 13 feet in height to the paved ground below when working from a scaffold that had 2 of a possible 4 planks installed while performing demolition work on a commercial building under renovation.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:    On or about May 4, 2015, employees were exposed to fall hazards of approximately 13 feet in height to the paved ground below from climbing the scaffold end frames and cross braces to access the scaffold while performing demolition work on a commercial building under renovation.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level:  On or about May 4, 2015, employees were exposed to fall hazards greater than 10 feet to the paved ground below when working on a scaffold without the use of a guardrail system while performing demolition work on a commercial building under renovation.

29 CFR 1926.451(h)(1): In addition to not wearing hardhats each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, or guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects:  On or about May 4, 2015, employees were exposed to a struck by hazards from falling siding, hammers, and crow bars while performing demolition work on a commercial building under renovation without hard hats from a scaffold that did not have toe boards installed.
