On May 5, 2015 and at prior times and possibly at times subsequent thereto, at the Martin Operating Partnership, L.P. Neches Terminal, in Beaumont, Texas, the employer did not ensure that the process safety information depicted in P&IDs was up-to-date and accurate for the Ammonia transfer process when:      (a) The employer P&ID NEC-02958 Rev G did not include detail to allow valves and instruments to be positively identified.      (b) The employer P&ID NEC-02962 Rev H did not provide information to represent pumps P-108B and P-108C that are part of the ammonia process.      (c) The employer P&ID NEC-02963 Rev G showed a pressure vessel PV-521 with maximum pressure of 250 psi being protected against over pressure by PSV-050 with set pressure at 270 psi.      (d) The employer P&ID NEC-02963 Rev G did not show a relief pressure for PSV-053      (e) The employer P&ID NEC-02967 Rev F did not provide accurate and complete information to represent Ammonia barge loading pumps P-106A and P-106B.      (f) The employer P&ID NEC-02968 Rev E did not include information to identify pressure vessels in the process.      (g) On the employer P&ID NEC-02969 Rev E, process safety information for V-4 is not included and several instruments on the diagram do not have identifying information.      (h) On the employer P&ID NEC-02972 Rev F, process safety information on the diagram is for equipment not represented on the diagram and there is no information on the diagram that allows identification of the PSV.      (i) On the employer P&ID NEC-02973 Rev F,  process safety information on the diagram is for equipment not represented on the diagram and there are several instruments that have no identifying detail included on the diagram.      (j) The employer P&ID NEC-02976 Rev D did not provide information to positively identify PSVs associated with the Oil Separator V-4 and Oil Cooler V-7.     (k) The employer P&ID NEC-02977 Rev D did not provide information to positively identify PSVs associated with V-5 and V-8.     (l) The employer P&ID NEC-02978 Rev D did not provide information to positively identify PSV associated with V-9.     (m) The employer P&ID NEC-02979 Rev D did not provide information to positively identify and verify the PSVs associated with V3 and does not include information to identify instruments on the diagram.     (n) The employer P&ID NEC-02982 Rev F did not provide identification numbers for several instruments in the control system for the ammonia heater.     (o) The employer P&ID NEC-02983 Rev E did not provide identification numbers for several instruments in the control system for the ammonia heater.      (p) The employer P&ID NEC-02988 Rev A did not provide identification number or relief pressure detail for the PSV shown in the diagram.

(b) External coatings of piping in the ammonia transfer system were not maintained.      (C) Process valves were installed in operating piping systems but did not have handles on the valves for operation (valves cannot be operated properly to control the process without handles or valve operators installed).
