On or about 05/04/15, on the first floor of 173-36 47th Ave. Flushing, NY Employees were cutting wood with a 10 Ryobi table saw without a guard. ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): A). On or about 05/04/15, at 173-36 47th Ave. Flushing, NY Employees working on the fabricated frame scaffolds in the back of the house were exposed to the hazard of falling approximately 13 ft. to the ground. The working platforms were installed with only two scaffold planks. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.

NY Employees applying stucco side of the house on the scaffolds were exposed to the hazard of falling approximately 13 ft. to the ground. Guardrails or personal fall arrest systems were not provided. ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
