29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Location: Pro Printing NY Printing shop located at 2961 Ely Ave., Bronx NY. a) As a United Graphic Systems, Inc. technician repaired a UV-Coating printing machine (JLG Model SGZ-UV740A), he and employees of host employer Pro-Printing NY who manually handled the wheel and pulley on that machine were exposed to the hazard of having their hands struck or pinched by those moving parts in the event of unexpected startup of the machine. United Graphic Systems, Inc. did not established a lock-out/tag-out program to protect employees against these hazards; on or about 02-19-2015.
