29 CFR 1910.178(m)(5)(iii): Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not have the load engaging means fully lowered, controls neutralized, and the brakes set to prevent movement: 235 South Monmouth Road - On or about 5/5/15 the load of the JCB 508 lift truck were elevated while an operator was not at the controls. The load included a welding machine on a pallet on the forks of the truck.
