29 CFR 1926.1408(a)(1): The employer failed to identify the work zone by either demarcating the boundaries (such as with flags, or a device such as a range limit device or range control warning device) prohibiting the operator from operating the equipment past those boundaries, or defining the work zone as the area 360 degrees around the equipment, up to the equipment's maximum working radius: (a) on the jobsite - employees involved in placing wooden mats were exposed to an electrocution hazard in that the crane work zone was not identified, resulting in the crane becoming energized, creating an electrical arc flash on or about April 19, 2015.

29 CFR 1926.1408(a)(2): The employer did not meet the requirements in Option (1), Option (2), or Option (3) of this section when any part of the equipment, load line or load (including rigging and lifting accessories), if operated up to the equipment's maximum working radius in the work zone, could get closer than 20 feet to a power line: (a) on the jobsite - employees involved in placing wooden mats were exposed to an electrocution hazard due to the ability of the crane boom to get within 20 feet of the energized 69 kV power line and not meeting the requirements of the encroachment prevention measures of paragraph (b), which resulted in the crane becoming energized, creating an electrical arc flash on or about April 19, 2015.
