29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports:    (a) Jobsite, McKeon Alley, Greensburg, PA:  On or about 05/05/15 the working platforms on all levels of the scaffold were not fully decked or planked.  One employee was working on the third level of the scaffold that only had three planks installed and another employee was working on the second level of the scaffold that only had one plank installed.  The fall hazards to the employees ranged between 11-19 feet.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:    (a) Jobsite, McKeon Alley, Greensburg, PA:  On or about 05/05/15 no ladder or other means of access was provided on one of the scaffold sections.  An employee climbed the scaffold frame to access the working area on the second level.  This condition exposed the employee to potential fall hazards of approximately 11 feet to the ground.      The employer, Raimondo, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(e)(1), which was contained in OSHA inspection number 554639, citation number 1, item number 1 and was affirmed as a final order on 09/17/12, with respect to a workplace located at 718 4th Avenue, Ford City, PA.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:    (a) Jobsite, McKeon Alley, Greensburg, PA:  On or about 05/05/15 an employee was working on the right scaffold section approximately 11 1/2 feet above the ground, on one plank, and no guardrails or personal fall arrest systems were in place to protect the employee from falling.      The employer, Raimondo, Inc., was cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1)(vii), which was contained in OSHA inspection number 554639, citation number 1, item number 3 and was affirmed as a final order on 09/17/12, with respect to a workplace located at 718 4th Avenue, Ford City, PA.      The employer, Raimondo, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number 995473, citation number 1, item number 1 and was affirmed as a final order on 11/25/14, with respect to a workplace located at 311 Washington Road, Pittsburgh, PA.
