29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. Employee was observed working from the upper fourth level of a walk through, tubular welded frame, scaffolding system without proper fall protection. Worker was exposed to potential 26.0 fall hazards in that a proper fall protection system was not utilized.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. Located on the upper level a tubular welded frame scaffolding system an employee was observed helping masons unload a bearing plate. The worker was exposed to potential 26.0 foot fall hazards in that the worker was not properly trained in recognizing hazards associated with scaffolding systems.
