29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. This violation occurred on or about May 4, 2015, at I-35 southbound HWY 9 exit, employees involved in the operation of moving a steel plate which was attached by an alloy steel chain to a Komatsu excavator were not adequately trained to properly rig the steel plate, exposing employees to the hazards of being struck by the load.

29 CFR 1926.251(a)(1): Defective rigging equipment was not removed from service. 1) This violation occurred on or about May 4, 2015, at I-35 southbound HWY 9 exit, the employer did not remove from service the Alloy Grade 80 Clevis Sling Hooks with missing safety latches that were used on the Grade 80 Alloy Steel chain in the Quadruple Chain Sling with Master Link and Sling Hook configuration. 2) This violation occurred on or about May 4, 2015, at I-35 southbound HWY 9 exit, the employer did not remove from service wire rope cable slings with bent and twisted safety latches. Employees were exposed to the hazards of being struck by falling loads.
