29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, or flying chips and sparks: (a) On or about May 7th, 2015, and at certain times before, employees were exposed to entanglement hazards from rotating parts in that the chuck on the LeBlond metal lathe (serial number 90-246 U-4709) was not adequately guarded. Abatement certification is required for this Item. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.212(a)(3)(ii): The points of operation of machines whose operations exposed employees to injury, were not guarded: (a) On or about May 7th, 2015, and at certain times before, employees operating the Sajo and Nichols horizontal milling machines, and the Sharp vertical milling machine, were exposed to severe injuries, including possible amputations, in that the points of operation on these machines were unguarded. 29 CFR 1910.219(e)(3)(i): Vertical or inclined belt(s) were not enclosed by guard(s) conforming to the requirements specified at 29 CFR 1910.219(m) and (o): (a) On or about May 7th, 2015, employees were exposed to possible injury from in-running nip points created by unguarded belts and pulleys. A side panel (guard) had been removed from the side of the Mori Seiki CNC Milling Machine (Model SL-25) which exposed employees to possible contact with the belts and pulleys inside the machine.
