29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) 1300 PERRY STREET, READING, PA - An employee performing roofing work was wearing a personal fall protection harness, but was not attached to an anchor point, thereby exposing the employee to a fall hazard that ranged from approximately 15 to 28 feet to the ground below, on or about April 29, 2015. No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) 1300 PERRY STREET, READING, PA - Employees used a ladder to access the roof where they were performing work, and the ladder only extended approximately twelve (12) inches above the upper landing surface, thereby exposing employees to possible fall hazards of approximately twenty-three (23) feet to the ground below, on or about April 29, 2015. No abatement certification or documentation required.
