29 CFR 1926.453(b)(2)(v):     A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under Sec. 1926.502(e).    Employees used a Skyjack SJ45T Telescopic Boom aerial lift to access a roof at 19.7 feet to perform repair work. The employees were not protected from ejection from the lift platform by a body harness or restraining belt with a lanyard attached to the boom or basket.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(4)(i):     Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    Employees were not protected from a fall through skylights while performing roofing operations at a commercial property, exposing employees to a fall in excess of 17 feet to the lower level.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
