29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: a. Waterbrooke Subdivision, 4810 Breezewood Circle, Alpharetta, GA (Jobsite) - Employee were exposed to fall hazard. An employee was installing sidings on a newly residential home while working on a ladder jack scaffold approximately 13 feet from ground level, without utilizing a means of fall protection, on or about May 07, 2015.

An employee is using a aluminum portable extension ladder, which was taken apart at sections, as a crawling chicken board ladder which was used as steps to gain access to an upper level side of the home, on or about May 07, 2015.
