29 CFR 1910.304(b)(3)(ii)(C)(3): Each cord set, attachment cap, plug, and receptacle of cord sets, and any equipment connected by cord and plug were not visually inspected before use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage, and equipment found damaged or defective was not removed from use until repaired: (a) At the Iowa City Highway 6 South facility the flexible cord from the case lighting was damaged in that the insulation had pulled away from the connector plug and had not been removed from service. The light cord was plugged into an extension cord.

29 CFR 1910.37(a)(3): Materials or equipment were placed, either permanently or temporarily, within the exit route: (a) The exit door next to the West dock doors was blocked on both sides by parcels and equipment. A sign was taped to the door that indicated the door was blocked and do not use. Above the door was a lighted EXIT sign.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: (a) At the Iowa City Highway 6 Facility, flexible cords were used as permanent wiring to provide power to lighting over two banks of driver cases located at the north east and north west ends of the case area. The US Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29CFR 1910.305(g)(I)(iv(A), which was contained in OSHA inspection number 31021660, Citation number 1, Item number 2 and was affirmed as a final order on March 13, 2015 with respect to a workplace located at 11111 Jefferson Blvd., Culver City, CA 90230.
