The scaffold was installed with a work platform which consisted of an extension ladder spanning approximately 30 feet and was not an adequate guardrail system.

Lifelines were attached to the triangular bracing used to support the top of the pump jack poles which supported the pump jack scaffold the employees were working from and did not provide a safety factor of at least two.
