29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) Throughout jobsite - On or about May 7, 2015, employees working on the ground level were not wearing protective helmets or hard hats and were exposed to head injuries from falling objects such as but not limited to pneumatic nail guns, hand tools, and pieces of siding being installed on a house.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) First Story Roof - On or about May 7, 2015, employees used an aluminum extension ladder to gain access to the roof of the house without the side rails of the ladder being extended at least 3 feet above the upper landing surface nor was the ladder secured at its top to a rigid support, exposing employees to fall hazards of up to approximately 10 feet onto earthen ground.
