29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: (a) Location - 49 Queenstown Street, Devens, MA: On or about 5/5/15, employees were not protected from trip hazards where holes in the floors of the large and small latrines on the first, second, and third floors were not protected with covers. Holes in floors of the large latrines on the second and third floors without covers had metal fittings placed in the holes for plumbing layout purposes and created a greater trip hazard.
