29 CFR 1926.800(r)(14): Parked rail equipment was not chocked, blocked, and did not have brakes set to prevent inadvertent movement: Along the interior north side end of the bottom tunneling railing dumping area, on or about May 05, 2015, and at times prior thereto, employees were exposed to a crush hazard from a 30 horse power diesel locomotive that was not secured to prevent movement while unloading soil and tunnel forms during tunneling operations.

29 CFR 1926.800(r)(16): Bumper blocks or equivalent stopping devices were not provided at all track dead ends: Along the south side end of the bottom tunneling railing track system area, on or about May 05, 2015, and at times prior thereto, employees were exposed to crush hazards from a 30-horsepower diesel locomotive that was being used to handle material such as soil and metal tunnel forms without having any type of stopping system device at the end of the railing track system. Southland Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 315441402 citation number 01, item number 006 and was affirmed as a final order on October 31, 2011, with respect to a workplace located at 1730 Lake Road, Webster, NY 14580.
