29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:    a)  On or about 05/08/2015, the employer did not take the Linde E20B electric forklift out of service while it was in an unsafe operating condition due to the truck's damaged and inoperable operator restraint system (seatbelt). Employees operating the Linde E20B would be exposed to caught between hazards in a tip-over event.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a)  On or about 05/08/2015, suitable facilities for flushing of the eyes were not provided within the work area for immediate emergency use when forklift operator employees were exposed to corrosive battery acid while charging and adding water to forklift batteries and when sanitation employees were exposed to corrosive cleaning chemicals during sanitation cleaning.
