29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) At 627 Walmart Drive in Farmington, MO - On or about 05/06/2015, employees were installing a white membrane roofing system on a low-slope (flat) roof, approximately 17 feet or greater above a lower solid concrete surface, without having any fall protection system in place, exposing the employees to elevated fall hazards. BOURDEAU CONTRACTING, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD (1926.501(b)(10)), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314989070, CITATION NUMBER 1, ITEM NUMBER 2 AND WAS AFFIRMED AS A FINAL ORDER ON 11/09/2010, WITH RESPECT TO A WORKPLACE LOCATED AT 3405 GRAVOIS AVE IN SAINT LOUIS, MO 63118.
