29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  a. On or about May 01, 2015, at the above addressed jobsite, five employees were installing a prefabricated concrete retaining wall, for which the shoring lumber/board, and the metal stake(s) (forming pin) were not inspected by a Competent Person for indications of failure, and for which the shoring lumber/board, and the metal stake(s) (forming pin) were not adequately secured/installed, which resulted in the concrete wall falling onto one of the employees, and therefore subsequently resulting in the serious injury of the worker.

29 CFR 1926.704(a): Precast concrete wall units, structural framing and tilt-up wall panels were not adequately supported to prevent overturning and to prevent collapse until permanent connections were completed:  a. On or about May 01, 2015, at the above addressed jobsite, an employee was installing the wooden wedges along the bottom of a prefabricated concrete retaining wall, which was not adequately shored/secured by the metal stake(s)/forming pins, and for which the wall was also not being supported by the lifting device/crane, until it would have been shored/secured, and wherein the concrete wall fell onto the employee, therefore resulting in the serious injury of the worker.
