29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. At the above worksite, employees working at or near the edge of the mezzanine level were exposed to fall hazards of approximately nine feet to the lower level due to an incomplete guardrail system. The employer did not ensure the employees were protected from fall hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: On or about May 4, 2015, at the above worksite, employee(s) working on the mezzanine level around an open floor hole was not protected from falling approximately nine feet to the lower level. The employer did not ensure that employees were protected from fall hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
