29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident: (a) Facility- 6000 North Noah, Saxonburg, PA: On or about April 14, 2015, an employee stuck another employee while operating their pallet jack, and the operator was not retrained on the operation of that pallet jack nor evaluated on the effectiveness of that training.
