29 CFR 1910.178(o)(1): Loads on powered industrial trucks were not stable or safely arranged: (a) Kettle Area - Employees operated forklifts with unstable loads in that employees moved skids containing drums without securing the drums to the skids. On or about April 17, 2015, a kettle employee operating a forklift raised a skid, containing four drums of hot sauce, about 7 feet above the floor and backed and turned the forklift with this load elevated. The operator did not have the drums of hot sauce secured to the skid, and one of the drums slid off the skid, landed on the floor, and released its contents of hot sauce. The hot sauce splashed onto another employee working in the area, causing this other employee to suffer burns to multiple parts of the body, including a third degree burn to the foot. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA: (a) Earth Pride Organics, LLC, dba Lancaster Fine Foods, 2320 Norman Road, Lancaster, Pennsylvania - An employee suffered a third-degree burn to the foot, which required in-patient hospitalization on April 17, 2015 as a result of getting splashed with hot sauce. The employer failed to report this work-related hospitalization to OSHA within 24 hours. (The employer reported this hospitalization to OSHA on May 6, 2015.)
