29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: (a) In the maintenance shop: On May 6, 2015 and at times prior to; the employer exposed his employees to struck-by hazards from a disintegrating stone from the 6 inch blue bench grinder operating at 3450 rpm, in that the tongue guard on the left side was adjusted to approximately 7/8 inch. (b) In the maintenance shop: On May 6, 2015 and at times prior to; the employer exposed his employees to struck-by hazards from a disintegrating stone from the Dayton 6 inch bench grinder operating at 3450 rpm, in that the tongue guard on the left side was adjusted to approximately 1 inch.
