29 CFR 1910.180(h)(3)(v): No hoisting, lowering, swinging, or traveling shall be done while anyone is on the load or hook. At the time of the inspection upon arrival at the site on or about 230pm 06 May 2015, at a building material delivery address of 2413 S. 10th St, Saint Louis, MO 63104, CSHO 7645 observed the lead man and senior truck driver onsite acting as company representative, approximately 20-30 feet in the air, straddling two Thermoplastic polyolefin (TPO) rolls that were approximately 15-20 foot in length. The TPO rolls were cradled by two slings and suspended from the crane as they were being lifted by the mobile truck mounted crane to the rooftop of a three story residential building with an approximate height of over 40 feet.
