29 CFR 1926.451(e)(9)(i): The employer did not provide safe means of access for each employee erecting or dismantling a scaffold. On or about 4 May 2015, and at times prior thereto, four scaffold builders were dismantling a QuikDeck Suspended Access System underneath an interstate highway bridge, in which a safe means of access was not provided between the suspended deck and the stair tower, exposing the employees to a fall hazard of approximately 30 feet to the ground below.
