29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a.  On or before May 12, 2015, the employer failed to ensure that effective guarding was in place on the input side of the Herr-Voss Stampco Slear 3 Line.  Barrier guards were not present to prevent employees from the ingoing nip point of the guide rollers.    b.  On or before May 12, 2015, the employer failed to ensure that effective guarding was in place on the output side of the Slear 3 Line.  Employees reach into the area where there are moving machinery parts such as the trolley and guide rollers.    c.  On or before May 12, 2015, the employer failed to ensure that effective guarding was in place on Slear 3 Line.  The middle section of the line guide rollers contained a large opening capable of allowing employees to enter the hazard area.      d.  On or before May 12, 2015, the employer failed to ensure that effective guarding was in place on Slitter Line 1.  The in-feed rollers did not contain a barrier guard on the non-operator side.      e.  On or before May 12, 2015, the employer failed to ensure that effective guarding was in place on Slitter Line 1.  The barrier guards in front of the closing rams on the output/straightening portion were not permanently affixed.  The guard could be opened by removal of a simple cotter pin.      f.  On or before May 11, 2015, the employer failed to ensure that effective guarding was in place on Slitter Line 2.  Barrier guards were not present on the in-feed rollers.      g.  On or before May 11, 2015, the employer failed to ensure that effective guarding was in place on Slitter Line 2.  The barrier guards in front of the closing rams on the output/straightening portion were not permanently affixed.  The guard could be opened by removal of a simple cotter pin.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a.  On or before May 12, 2015, the employer failed to ensure that effective guarding was in place on the Slitter Line 1.  The barrier guard on the slitting rolls section of the line were interlocked but not large enough to prevent employees from going over, under or around the guard.    b.  On or before May 11, 2015, the employer failed to ensure that effective guarding was in place on the Slitter Line 2.  There were no barrier guards at the shear point where employees were within 20-27 inches of the point of operation hazard.    c.  On or before May 11, 2015, the employer failed to ensure that effective guarding was in place on the Slitter Line 2.  The barrier guard on the output side of the Slitter Line was not effective in prohibiting employees from going over, under or around the guard.
