29 CFR 1910.305(c)(4): Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface.     a) Manager Office, 501 West Main Street, Plymouth, PA - The flush-mounted snap switch (light switch) that operated the fluorescent lights in the Manager's Office lacked a cover and had exposed energized conductors, exposing employees to a contact with electrical current hazard, on or about May 8, 2015.

29 CFR 1910.305(j)(1)(i): Fixtures, lampholders, lamps, rosettes, and receptacles located within 8 feet of the floor had live parts exposed to employee contact:  a) Stock Room, 501 West Main Street, Plymouth, PA - A 120-Volt dual-receptacle wall outlet with a broken receptacle and exposed energized conductors was located within thirty-eight inches (38") of the floor, exposing employees to a contact with electrical current hazard, on or about May 8, 2015.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment.    a) Store Room, 501 West Main Street, Plymouth, PA - Boxes containing merchandise, empty plastic containers and two (2) appliances upon a wooden cart were stored in front of electrical equipment in the storage area, impeding the ready and safe operation and maintenance of the equipment by employees, as observed on or about May 8, 2015.    Family Dollar Store, Incorporated was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard, 1910.303(g)(1)(ii) which was contained in OSHA Inspection Number 560098.015, Citation Number 1, Item Number 1 issued on December 20, 2012 and was affirmed as a Final Order on January 17, 2013, with respect to a workplace located at 1901 Greenwood Avenue, Trenton, NJ.
