29 CFR 1926.501(b)(2)(i): Each employee who is constructing a leading edge 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems. On 5/06/2015, an employee from Fastrack Erectors was working on a steel beam on a leading edge of a roof of a commercial building located at Frontier Plaza in O Fallon MO. The employee was working approximately fourteen feet above the ground without the proper use of guardrail systems, safety net systems, or personal fall arrest systems.
