29 CFR 1926.451(g)(3): The employer did not meet the requirements of 1926.502(d) in its use of personal fall arrest systems used on scaffolds:    a)  Rear of Building - On or about 05/08/2015, two workers were installing Tyvek wall covering from the platform of a pump jack scaffold, which the employer had not determined to have a safety factor of two as required by 1926.502(d)(8).

29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, and did not determine that the system maintained a safety factor of at least two:    a)  Rear of Building - On or about 05/08/2015, two workers were installing Tyvek wall covering from the platform of a pump jack scaffold were exposed to a 46 foot fall where the horizontal lifeline, which was part of their fall protection system had not been determined to have a safety factor of at least two by a qualified person.
