29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  a) Residential jobsite: On May 8, 2015 and at times prior thereto an employees performed framing activities on unsheathed trusses, and on the exterior top plate, without a fall protection system in place.  Employees were exposed to a 19 foot fall hazard to concrete and the ground below.    b) Residential jobsite: On May 8, 2015 and at times prior thereto a guardrail system was not in place around the unprotected edges of the stairway floor opening on the second floor.  Employees were exposed to an 8 and a half foot fall hazard to the lower level.       c) Residential jobsite: On May 8, 2015 and at times prior thereto guardrail systems were not in place for window and wall openings on the second floor. Employees were exposed to a 13 foot fall hazard to concrete and the ground below.    Note) Certification and documentation of corrective actions ARE required for these items

29 CFR 1926.503(a)(2)(iii): The employer did not ensure that each employee had been trained as necessary, by a competent person qualified in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:   a) Residential jobsite: On May 8, 2015 and at times prior thereto employees did not recognize or acknowledge the hazards of performing framing activities above 6 feet without any form of fall protection in place.  Nor were the employees aware of any actions that could be taken to correct the hazards or minimize their exposure to the hazards.   b) Residential jobsite: On May 8, 2015 and at times prior thereto employees did not recognize or acknowledge the hazards of walking and working near an unprotected floor opening.  Nor were the employees aware of any actions that could be taken to correct the hazard or minimize their exposure to the hazard.    c) Residential jobsite: On May 8, 2015 and at times prior thereto employees did not recognize or acknowledge the hazards of walking and working near unprotected window and wall openings.   Nor were the employees aware of any actions that could be taken to correct the hazards or minimize their exposure to the hazards.  Note) Certification AND documentation of corrective actions ARE required for these items.
