29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person:  On or about 05/06/2015, at the job site at 7076 Brancroft Street, St Louis, MO, employees engaged in roofing operations were exposed to hazardous conditions created by the employers failure to conduct  frequent or regular inspections.  The employees were exposed to an approximate 13foot fall hazard while working on the roof without wearing a harness that had been properly tied off to an anchor point.  Also, they were exposed to struck-by hazards as a result of using a nail gun without required eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  On or about 05/06/2015 at the job site at 7076 Brancroft Street, in St Louis, MO, employees working on a steep roof engaged in installing roofing shingles without a personal fall arrest system.    The ground to eave height was approximately 13 feet.

29 CFR 1926.503(a)(1):   The employer does not ensure that a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about 05/06/2015, at the jobsite at 7076 Bancroft Street, St Louis, MO, an employee was exposed to an approximately 13 foot fall.    Fall protection was not utilized and the employer failed to provide the employee with fall protection training.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, shall either be immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired.  On 05/06/2015, employees working for Valenti Roofing at 7076 Bancroft Street, St Louis(work site) were using a defective 16 foot portable extension ladder, to access a residential building to repair the roof.
