OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    a) At the facility- On or about May 09, 2015, the employer did not provide a safe way to mount/dismount the fixed ladder by the seed pit/front of the control panel leading to the pit/basement area, exposing employees to fall hazards of up to 17 feet onto the ground.       b) At the facility- On or about May 09, 2015, the employer did not provide a safe way to mount/dismount the fixed ladder by the first stage cleaning area leading to the pit/basement area, exposing employees to fall hazards of up to 6.5 feet onto the ground.      c) At the facility- On or about May 09, 2015, the access points into the pit/basement area were not adequately guarded when employees were working in the upper level, exposing employees to fall hazards of up to 17 feet onto the ground.
