29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about May 07, 2015, employees were exposed to struck-by hazards from items such as but not limited to: hammers and sharp sheet metal, when not wearing protective helmets, while other employees were working above them on the roof installing the sheet metal roofing to the residential home.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about May 07, 2015, employees were exposed to fall hazards greater than 6 feet to the ground level below, when working on the roof of a residential home, not wearing safety harnesses for fall protection, while installing the sheet metal roofing to the home.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about May 07,2015, employees were exposed to falls greater than 6 feet to the ground level below, when using a 20 feet Werner extension ladder and the side rails were 6 inches above the landing, while accessing and egressing the roof of the residential home.
