29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about May 7, 2015, at 30 Baltruso Way, Covington, Georgia:  Two employees performing framing work were exposed to struck-by hazards from falling nail guns, wood, and other materials of the employees working above them when the employees working from the ground below did not use head protection.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about May 7, 2015, at 30 Baltruso Way, Covington, Georgia:  a) An employee performing framing work was exposed to struck-by hazards from nails and wood chips while operating a nail gun without using eye protection.   b) An employee performing framing work was exposed to struck-by hazards from wood chips and saw dust while operating a Makita 5007F circular saw without using eye protection.

29 CFR 1926.300(b)(1): Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use:  On or about May 7, 2015, at 30 Baltruso Way, Covington, Georgia:  An employee performing framing work was exposed to a laceration and amputation hazard while using a Makita 5007F circular saw that had a piece of wood wedged next to the saw blade to prevent the lower blade guard from engaging.
