29 CFR 1910.94(a)(4)(i): The construction, installation, inspection, and maintenance of exhaust systems did not conform to the principles and requirements set forth in the American National Fundamentals Governing the Design and Operation of Local Exhaust Systems, Z9.2-1960, and ANSI Z33.1-1661, which is incorporated by reference as specified in CFR 1910.6: Abrasive Blasting Room: The ventilation system was automatically shutting off while employees were blasting. Employees were exposed to injury from steel shot and limited visibility from excess dust. Employer did not ensure that the exhaust ventilation was maintained in good condition.

29 CFR 1910.134(h)(3)(ii)(A): Respirator inspections did not include a check of respirator function, tightness of connections, and the condition of the various parts including, but not limited to, the face piece, head straps, valves, connecting tube, and cartridges, canisters or filters: Abrasive Blasting Area: The employer did not ensure that respirator inspections included a check of the condition of the hoses of the Apollo airline respirator used by employees for protection against particulate and metal dust during abrasive blasting of metal frames. The hose that supplies breathing air to the Apollo Airline respirator face piece was damaged, exposing employees to particulate and metal dust.
