29 CFR 1910.132(e): The employer did not ensure that employees were not using defective or damaged personal protective equipment in the workplace: On May 5, 2015, at the TimkenSteel Harrison Plant tundish repair area, the elevated platform had a Miller retractable fall arrest system that was damaged and did not retract. Employees were exposed to a fall hazard of approximately 9 feet while performing the task of dumping used refractory material from the tundish assembly to the work floor below.
