29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Building 6, Mallard Court, Flemington, NJ 08822. Employee was exposed to struck by hazards when working at ground level under active roofing work while not wearing any head protection, i.e. hard hats.      Condition occurred on or about 05/11/15.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a) Building 6, Mallard Court, Flemington, NJ 08822. Employee was exposed to eye and face injuries from flying debris when operating a pneumatic nail gun and not wearing any eye and or face protection.    Condition occurred on or about 05/11/15.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:    a) Building 6, Mallard Court, Flemington, NJ 08822. Employee was exposed to fall hazards when placing and installing new roofing materials with no fall protection systems in place.     Condition occurred on or about 05/11/15.
