29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i. when dead-ended: (a)Gulfeagle Supply at 11060 Leroy Drive, North Glenn, CO 80233: On and before May 12th, 2015, the employer did not ensure that compressed air used to clean off forklifts was used with an approved nozzle that reduced the p.s.i. to less than 30 when the nozzle was dead-ended. This condition exposed employees to an embolism and flying debris.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: (a)Gulfeagle Supply at 11060 Leroy Drive, North Glenn, CO 80233: On and before May 12th, 2015, the employer allowed the use of the 120 volt Speedaire air compressor, located in the northeast corner of the warehouse, without ensuring that it was properly grounded. The compressor motor power cord was missing the ground prong. The condition exposed employees to electrical shock hazards.
