29 CFR 1910.1030(d)(4)(i): The employer did not determine or implement an appropriate written schedule for cleaning or method of decontamination based upon the location within the facility, type of surface to be cleaned, type of soil present, and tasks or procedures being performed in the area:  (a)   Establishment: Employees including but not limited to Medical Assistants have exposure to blood and other potentially infectious material in that they are responsible to clean the exam rooms in between patients, but a written schedule  for cleaning had not been developed on or about May 8, 2015.

29 CFR 1910.1030(g)(2)(i): The employer shall train each employee with occupational exposure in accordance with the requirements of this section. Such training must be provided at no cost to the employee and during working hours. The employer shall institute a training program and ensure employee participation in the program. 29 CFR 1910.1030(g)(2)(ii): Training shall be provided as follows: 29 CFR 1910.1030(g)(2)(ii)(A): At the time of initial assignment to tasks where occupational exposure may take place; 29 CFR 1910.1030(g)(2)(ii)(B): At least annually thereafter.  (a) Establishment: Employees including but not limited to Medical Assistants have exposure to blood and other potentially infectious material in the course of their work, but annual training regarding the Exposure Control Plan was not conducted, thus potentially exposing employees to bloodborne pathogens, on or about May 8, 2015.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.  (a) Establishment: Employees including but not limited to Medical Assistants have exposure to hazardous chemicals in the course of their work, but annual training regarding the Hazard Communication Program was not conducted, thus potentially exposing employees, on or about May 8, 2015.
