29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    (a) Charleston: An employee was exposed to a fall of up to 21 feet to the ground while working on the soffit on the dormer on a 7:12 pitch roof, as observed on or about May 12, 2015.      ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM.

(a) Charleston: Employee of Shield Roofing, LLC. was utilizing a ladder to access the roof that was approximately 21 foot from eave to ground that did not have the ladder side rails extended at least 3 feet above the upper landing surface, as determined on May 12, 2015.
