29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  At the time of inspection, the employer failed to establish an energy control procedure for machinery, including, but not limited to, the Auto Spray Machine in the Spray Room, when employees are tasked with removing and cleaning the belt. Employees were expected to turn off the electrical cut-off switch, that would shut off all electronically and mechanically controlled energy sources.

29 CFR 1910.178(l)(4)(i):  Refresher training, including an evaluation of the effectiveness of that training, shall be conducted as required by paragraph (l)(4)(ii) to ensure that the operator has the knowledge and skills needed to operate the powered industrial truck safely.  At the time of inspection, the employer failed to provide the operators of the Mitsubishi FG025 fork truck, serial number AF 12B-01, refresher training every three years. The employee's certification had expired on March 15th, 2015.

29 CFR 1910.213(p)(1): Feed rolls of self-feed sanding machine(s) were not protected with semicylindrical guard to prevent the hands of the operator from coming in contact with the in-running rolls:  At the time of inspection, the employer failed to provide guarding to the wood brush sander. The rollers were exposed creating a nip point.

29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by approved cabinets or other forms of approved enclosures, or other means listed under this provision:   At the time of inspection, the employer failed to guard live electrical parts of equipment, including, but not limited to the SCMI SI 350N slide saw, serial number AB/172042. The control button panel had 3 holes with live electrical parts inside.

29 CFR 1910.305(b)(2)(i):  All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  At the time of inspection, the employer failed to provide covers to junction boxes including, but not limited, to the junction box located in the finishing room, near the fire extinguisher, and above the cordless drill battery charger.
