29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  (a)  In the manager's office:  On May 12, 2015 and at times prior to; the employer exposed his employees to electrical shock hazards in that employees were allowed to walk and work around an electrical outlet that was not protected by a face plate leaving conductors exposed.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a)  Facility wide:  On May 12, 2015 and at times prior to: the employer failed to conduct effective hazard communication training.
