29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. a) Site - On or about 05/13/15 and at times prior thereto employees performed carpentry/framing work and operated pneumatic nail guns and portable electric circular saws. Safety glasses were not in use, and were not available at the worksite. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Site - On or about 05/13/15 and at times prior thereto employees were installing panels on the roof of a two story residential home. Harnesses were in use, but were not connected to the lanyards. Employees were exposed to 20 ft. falls to ground level. (Roof slope, side 8:12). Roof slope, front: 12:12). In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification.
