29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On or about May 13, 2015 the point of operation on the heat seal press was not guarded in that the two hand control and relays were not safety-related control devices, exposing employees to a burn and caught-in hazards.
