29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On or about at 5/13/15 located at 3390 Riverwalk Way, Cumming, GA: Employees were framing a house that was under construction while on the second floor 2x4 walls and lumber were swung  overhead as employees were working below, without a hard hats exposed to a struck by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about at 5/13/15 located at 3390 Riverwalk Way, Cumming, GA: Employees were installing 2x4 walls on the second floor of a house that was under construction while working at the edge, the employer did not ensure that fall protection was used by the employees while exposed to fall hazards of approximately up to 14 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about at 5/13/15 located at 3390 Riverwalk Way, Cumming, GA: Employees were exposed to fall hazards, while performing framing work, employees were using a portable extension ladder to gain access to the 2nd floor, and the ladder was only 1 foot above the landing surface.
