29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 05/13/2015 employees at the Jonathan Lane construction project were exposed to approximately 12' falls.  Employees were engaged in framing activities without fall protection.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  On or about 05/13/2015 employees at the Jonathan Lane construction project were exposed to approximately 12' falls from unprotected window openings.  Employees were engaged in framing activities without fall protection.

Main stairway was not equipped with a stair rail system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about 05/13/2015 employees at the Jonathan Lane construction project were exposed to approximately 8' falls.  Employees ladder did not extend at least three (3) feet above the landing.
